Services
Care Coordinators
At
Friends Life Care, personal relationships mean everything, especially
when it comes to good caregiving. Unlike long-term-care insurance, Friends
Life Care arranges for and supplies your care. This helps us meet your individual needs with the
best services possible.
When you enroll in the Friends Life Care Plan, a Care Coordinator will
visit you in your home to get to know you — and your surroundings — long
before you ever need us. Your Care Coordinator keeps track of your current
health conditions, the medications you take, your living will and advanced
directives, and any other important information we might need in the event
of an emergency. Then, any time you require assistance, you simply call your
Care Coordinator. He or she will select and schedule the details of your
care, so you can recuperate worry free. If you need to reach us after hours,
we have Care Coordinators on call 24/7.
Home Health & Support Services
-
Home Health Aides — These are
the caregivers we send to assist you with daily living activities, such
as bathing, dressing, eating, moving about, toileting and continence
issues.
-
Registered Nurses and Licensed
Practical Nurses — At the direction
of your physician and in conjunction with your health-care benefits, we
obtain and coordinate appropriate medical services in your home.
-
Emergency Response Service — Members receiving care and members at risk are eligible to receive a LifeFone® medical alarm. Other members can purchase a LifeFone subscription for a small additional fee. With the touch of a button, LifeFone allows you to reach emergency services around the clock, wherever you are. For inquiries, e-mail jstorman@friendslifecare.org.
-
Nutritional Support — We provide
services like grocery shopping and food preparation (in accordance with
any dietary restrictions or prescriptions) when you are unable to prepare
meals for yourself.
-
Adult Daycare — If needed, we can arrange for
high-quality daycare to help prevent placement in a nursing home or an assisted
living facility.
-
Periodic Home Safety Review — Taking
any limitations you may have into account, we come to your home periodically
to help you identify and correct safety issues.
Nursing Home & Assisted Living
Friends Life Care makes every effort to help members stay at home for as long as safely possible;
however, there are occasions when members require the specialized care of
a nursing home or an assisted living facility (a residence for people who
need minimal medical care, but who require help with bathing, dressing, mobility,
etc.). Depending on the plan you choose, we will pay your selected daily benefit to the nursing home or assisted living facility of your choice.
Care That Moves with You
Your membership in Friends Life
Care is portable. In other words, if you require our services while
traveling or visiting anywhere in the U.S., or if you relocate to another
state, you carry the plan with you.
Friends Life Care provides the same level of care, and all the services
remain the same for the same fees. We locate and coordinate qualified caregivers,
according to our uncompromising standards, wherever you go.
Referral Services
In addition to our health-related services, Friends Life Care can refer you to reliable professionals and service providers whose references have been thoroughly checked and who receive consistently high marks from our members. If you need work done around the house or help with your personal affairs, Friends Life Care maintains a list of prescreened individuals and companies you can hire (at your expense) for the following:
- Home Maintenance
- Housekeeping
- Snow Removal
- Lawn Services
- Legal Services
- Financial Planning
